UPMC Enrollment

When you are ready to enroll in UPMC medical, dental, and/or vision benefits, please print out the form below, fill it out, and provide it to Andreas Beck.

2025 Company Medical Benefit Contribution

Beyond Spots & Dots will contribute up to the following amounts towards medical plans on a monthly basis based on the number of years of employment with the company.

Years of Employment

  • 0-1 Year: $205.00
  • 1-2 Years: $205.00
  • 3+ Years: $295.00

The medical benefit begins on the first day of the following month they are submitted, due to enrollment processing. For example: Sue submitted enrollment paperwork on February 15th. Sue’s Medical benefits begin at the beginning of the following month, in this case March 1st.

Company contribution levels will increase on the first day of the following month of your anniversary date. For example: Sue joined the company on February 15th and is celebrating her 3 year anniversary. Sue is eligible for the higher contribution rate starting on March 1st.

No reimbursements will be made for health coverage through other plans or carriers.

Medical plan contributions will be pre-tax deductions on payroll.

Plan Eligibility

Full-Time Employees are eligible post sixty (60) day new-hire period.

UPMC Account Login

Please sign up with the following website to view your coverage and print temporary health plan ID cards if necessary. If you have issues logging in, please contact UPMC.

Termination of Employment

Upon termination of employment, the Beyond Spots & Dots medical benefit contribution amount will be prorated for the month, based on the termination date.
